Book this appointment if you have a NEW CONDITION OR INJURY for which the doctor has NOT treated you before. For example, you have been treated for neck pain, but now have low back pain. Please note: Additional paperwork must be completed before your appointment. Allow extra time to accommodate documentation, examination, and possible x-rays if needed.
